In the video they seem to capo up to the fourth fret but here are the chords without a 
as it can be annoying having to put a capo on each time.

Note that the F# is played in the third position like this:

e|-6-B|-7-G|-6-D|-8-A|-x-E|-x-

The hammer on effect on the F# can be achieved by moving the first finger to the 4th 
on the e and then hitting the 6th fret again with the middle finger like this:

   ↓ ↑ ↓  ↓ ↓e|-6--6--6--4--6-|B|-7--7--7--7--7-|G|-6--6--6-------|D|-8--8--8-------|A|---------------|E|---------------|

To incorperate the melody, quickly before playing the G#m play:

e|--7/9--7--6--7--|G|----------------|D|----------------|A|----------------|E|----------------|

Also the B and Bm should be played on the 7th Fret.
