Start real slow, just keep repeating it, and it will make sense. The woodwinds go through the progression once, then Sufjan's guitar fades in:

The 4s on the G string are sort of optional, I can't really tell if he plays them or not, and it's a hell of a lot easier if you don't. Another verse follows, same guitar part, except now he adds some hammering for flare:
